This program lets you make a memorial contribution to the Veterinary Care Fund in a pet&#8217;s name and send a heartfelt note to the family \u2014 a meaningful way to acknowledge their loss while supporting other pets in need.<\/p>\n<p class=\"hp-callout\"><strong>Suggested contribution:<\/strong> $25 per memorial<\/p>\n<h3>How It Works<\/h3>\n<p>When you submit the form below, we&#8217;ll take care of three things on your behalf:<\/p>\n<ol class=\"hp-steps\">\n<li><strong>A draft email for you to send.<\/strong> We&#8217;ll prepare a message you can review, personalize, and send directly to the pet&#8217;s family.<\/li>\n<li><strong>A free memorial wall code for the family.<\/strong> The owner can use it to post a tribute on our <a href=\"https:\/\/nysvms.org\/foundation\/legacy-honorees\/\">Legacy Wall<\/a> at no cost.<\/li>\n<li><strong>An optional sympathy card by mail.<\/strong> If you&#8217;d like, we&#8217;ll mail a card to the family on your behalf, with your hospital as the return address.<\/li>\n<\/ol>\n<h3>Privacy &amp; Confidentiality<\/h3>\n<p>We&#8217;ve designed this program to protect everyone involved \u2014 you, your client, and their pet&#8217;s memory.<\/p>\n<ul class=\"hp-privacy\">\n<li><strong>Nothing is public without consent.<\/strong> No information appears on the Legacy Wall until the pet owner approves it.<\/li>\n<li><strong>Mailing details are never kept.<\/strong> If you ask us to send a card, the name and address are used solely to mail it and then deleted from our records.
