How to honor a patient
Thank you for choosing to remember a beloved patient. This program lets you make a memorial contribution to the Veterinary Care Fund in a pet’s name and send a heartfelt note to the family — a meaningful way to acknowledge their loss while supporting other pets in need.
Suggested contribution: $25 per memorial
How It Works
When you submit the form below, we’ll take care of three things on your behalf:
- A draft email for you to send. We’ll prepare a message you can review, personalize, and send directly to the pet’s family.
- A free memorial wall code for the family. The owner can use it to post a tribute on our Legacy Wall at no cost.
- An optional sympathy card by mail. If you’d like, we’ll mail a card to the family on your behalf, with your hospital as the return address.
Privacy & Confidentiality
We’ve designed this program to protect everyone involved — you, your client, and their pet’s memory.
- Nothing is public without consent. No information appears on the Legacy Wall until the pet owner approves it.
- Mailing details are never kept. If you ask us to send a card, the name and address are used solely to mail it and then deleted from our records. We act as your agent in handling this confidential information.
- Easy, secure access for owners. Families receive a password-less link to return to the site whenever they wish.
- Respectful follow-up. If an owner chooses to contribute or opt in for updates, they’ll hear from us no more than twice a year.
